Swallowing Exercises in Game Form

SilverFit Rephagia is designed to treat dysphagia by combining traditional swallowing therapy with cues and visual feedback on swallowing movement, strength, timing, and coordination. Using sEMG electrodes, muscle activity just below the skin surface becomes visible. Game visualizations on screen provide valuable biofeedback, motivating both client and therapist.

The SilverFit Rephagia allows better explanation of a swallowing exercise through visual cues on the screen in the form of a graph or a moving game. The biofeedback allows the therapist to provide more targeted guidance during treatment, so that complex exercises are taught more efficiently (Galek, 2020 & Galek 2022). The game element challenges the client to do more repetitions and practice more often. The importance of swallowing exercises

Swallowing problems in neurological disorders and intellectual disability

Swallowing problems can reduce eating pleasure and often lead to anxiety, embarrassment and social isolation. In addition, dysphagia is a major risk factor for problems such as malnutrition, dehydration, pneumonia and death. Dysphagia is common in the elderly with neurological disorders and people with intellectual disabilities, with 30% of nursing home residents and 85% of people with intellectual disabilities in Europe experiencing it (Richtlijnenlangdurigezorg.nl). Function-oriented swallow training programs aimed at, for example, strengthening swallowing strength and strengthening hyolaryngeal lift (“Mendelsohn maneuver”), are recommended by the Federation of Medical Specialists as a treatment of dysphagia. In addition, the Federation of Medical Specialists recommends the use of surface EMG (sEMG) as biofeedback to support active swallowing rehabilitation. The guidelines show that there have been several studies on the added value of sEMG during exercises. From these it was concluded that the technique is well applicable (Richtlijnendatabase.nl).

In the pharyngeal phase, exercises are provided to train normal swallowing, powerful swallowing, swallowing timing, controlled swallowing (skill-based swallow), and the Mendelsohn Manoeuvre. Biofeedback helps clients master these techniques and aids therapists in their explanations.

In the oral phase, SilverFit Rephagia offers exercises for tongue training, such as pressing against resistance and retraction. Additionally, exercises for jaw clenching/relaxation and straw suction are provided.

Each exercise, method, and visualisation includes explanations, giving therapists tools to create tailored exercises for each client.

Visualisations help clients understand and perform swallowing exercises effectively. Even clients with reduced cognitive function or moderate-to-severe intellectual disabilities can engage in the exercises. The interactive exercises motivate clients to practice several times a day, with both therapist and client gaining insight into progress.

Flexible Usability

SilverFit Rephagia includes a laptop, electrodes, and wireless sEMG sensors, making it usable during regular on-site treatments as well as at home or in hospital beds. The system is ideal for intramural treatments and works well alongside the SilverFit Flow module, which can be added to the SilverFit Rephagia.
